The Zimmer Revitan® femoral revision stem was being implanted in a patient. During the procedure, a small ring, part of the proximal part of the trial stem, was left on the distal part of the definite stem. This is a critical step in the implantation process as failure to remove the small ring can result in problems with the top screwcap of the proximal part of the definite stem, which can compromise its stability. The small ring left behind prevented the top screwcap from being tightened properly. Despite the disturbance, the stem was thought to be stable at that time.

However, two and a half years later, the situation became unstable when the proximal part of the stem fractured, leading to a revision surgery. This highlights the importance of ensuring that all parts are securely fastened during the implantation process and every step of the surgical procedure is carefully monitored.
