Minimally Invasive Internal Fixation for Unstable Pelvic Ring Fractures

Pelvic ring fractures can be caused by high-velocity trauma and can lead to significant morbidity and mortality. These fractures can be treated with open reduction and internal fixation (ORIF), but this approach is associated with high blood loss, long operation times, and prolonged hospital stays. Minimally invasive internal fixation (MIIF) is an alternative approach that can reduce these complications.

A study published in the Journal of Orthopaedic Surgery and Research evaluated the outcomes of MIIF with or without sacroiliac screws for unstable pelvic ring fractures. The study included 27 patients with an average age of 37.4 years who were followed up for a mean of 22 months. The MIIF procedure resulted in an average operation time of 129 47 minutes and a median intraoperative blood loss of 100 mL, which is significantly lower than that of ORIF.

All patients achieved bony union with an average union time of 13.3 weeks. The fracture reduction quality was evaluated using the Matta score, pelvic deformity index (PDI), and pubic symphyseal width (PSW). The MIIF procedure resulted in a significant improvement in PDI and PSW. In addition, functional recovery and general quality of life were evaluated using the Majeed score and the 12-Item Short-Form Survey (SF-12), respectively. The SF-12 physical and mental health scores were 45.1 10.2 and 53.2 6.3, respectively.
